1. Inherited Migraine, Familial Hemiplegic Migraine A familial hemiplegic migraine is a form of migraine headache that runs in families. Symptoms include insomnia, intense throbbing pain in one area of the head, often accompanied by nausea, vomiting, and extreme sensitivity to light and sound. Patients may also experience headaches or insomnia or both symptoms. These recurrent headaches typically begin in childhood or adolescence and can be triggered by certain foods, emotional stress, and minor head trauma. Each headache may last from a few hours to a few days. This type of migraine can also give rise to other symptoms such as insomnia and a loss of balance with or without a headache.
Currently, there are four genes that were found related to familial hemiplegic migraine: CACNA1A, ATP1A2, SCN1A, and possibly PRRT2. These genes are involved in the control of the transportation of ions like sodium, calcium, and potassium across the nerve cell membranes. Imbalances of these ions disrupt the release and uptake of neurotransmitters such as glutamate and result in increased glutamate levels in the brain. This causes over-excitability of nerves as well as nerve inflammation causing migraine headaches and other related symptoms.2
2. Vestibular migraines A vestibular migraine is a nervous system problem that causes repeated dizziness (or vertigo) that comes and goes in people who have a history of migraine symptoms. Unlike traditional migraines, the patient may or may not always have a headache and it is usually on one side of the head when they experience the headache. Other symptoms patient may experience include vestibular or balance problems, nausea and vomiting, motion sickness or sensitivity to sound, light or smell. Approximately 40% of migraine patients have some accompanying vestibular syndrome.
The pathology of vestibular migraines isn’t fully understood. But the research has showed that vestibular migraines are a combination of altered vascular and neural processes. It results from the misfires between nerve cells in the brain with cerebral spreading depression of Leão (a spontaneous spreading of an electrical charge along the cortex) followed by activation of pain receptors located in the brainstem which is not far from the vestibular apparatus. The release of neurotransmitters causes the dilation of blood vessels near the scalp and other structures outside of the brain substance.
Similar to other types of migraine syndromes, vestibular migraines tend to run in families. Women tend to suffer more from the condition than men, and symptoms may get worse around menstruation.
Both the inherited migraine and vestibular migraine are referred to as a Meridian Cold Wind in TCM.
