The lymphatic circulation of the lungs is important to maintain healthy lung function and immune surveillance. Lymphatic dysfunction or blockage can cause accumulation of metabolic toxins and affect the lung’s immune function. Patients may experience symptoms of chest pain, tightness and fullness, excessive cough and phlegm, rib cage pain, lung inflammation and easy to catch lung infections.
The lung’s lymphatic network consists of small, thin-walled, and blind-ended initial lymphatics in the parenchyma that collect and drain lymph into larger collecting lymphatic vessels around blood vessels and airways. These collecting lymphatics then drain the lymph to thoracic lymph nodes, and eventually into the thoracic duct to join the blood circulation.
The lungs are susceptible to inflammation and injury, as they are constantly exposed to pathogens, environmental toxins, and inflammatory stimuli. Lymph nodes in the lungs play a central role in coordinating the immune response to infection and inflammation when lymph fluid is drained to the lymph nodes. Lymphatic dysfunction has been found to be involved in many lung diseases. In some settings, lymphatic dysfunction may be secondary to lung injury, but in other settings, lymphatic dysfunction plays a causative role in the initiation and progression of lung disease including COPD, interstitial lung disease, sarcoidosis, and asthma.1
Animal studies have found that mice with impaired lymphatic function develop many of the histologic and pathologic hallmarks of human COPD, including hypoxia, formation of lung lymphoid follicles, and airspace enlargement due to the breakdown of elastin. Cigarette smoke exposure causes impaired lymphatic drainage, decreased leukocyte trafficking, and prothrombotic lymph resulting in lymphatic thrombosis. Lung lymphatic thrombosis is seen in human COPD, and is correlated with disease severity.
